1. Program Selection:

The Athlete has selected the following training program (check one): *

2. Program Details

Starting 5: Up to 2 on-court training sessions per week, one mental conditioning mentorship meeting per month, a 5-month commitment, $599/monthly.

Elite 8: Up to 3 on-court training sessions per week, mental conditioning mentorship meetings, private reports after each session, 2 game film breakdowns per month, an 5-month commitment, $899/monthly.

The All In: Up to 4 on-court training sessions per week, mental conditioning mentorship meetings, private reports after each session, 2 game film breakdowns per month, at least one in-person game visit per month, 2-4 pieces of marketing content monthly, a 5-month commitment, $1099/monthly.

3. Term and Termination:

The term of this Agreement shall commence on [Start Date] and shall continue for the duration specified in the selected program.

The Athlete may terminate this Agreement by providing 30 days written notice to the Academy. However, any fees paid are non-refundable, and early termination may incur additional fees.

4. Payment:

Payment is due monthly in advance, on or before the [Due Date] of each month.

The Athlete authorizes charges to be made every 30 days per the sign-up date.

Failure to make timely payments may result in suspension or termination of services.

No refunds will be provided under any circumstances.

5. Liability:

The Athlete acknowledges that participation in athletic training involves inherent risks, including the risk of injury. The Athlete voluntarily assumes all risks associated with participation.

The Academy shall not be liable for any injuries or damages sustained by the Athlete during training sessions, games, or other related activities.

The Athlete agrees to hold the Academy, its trainers, and staff harmless from any claims, demands, or causes of action arising out of or connected with the Athlete's participation.

6. Commitment:

The Athlete commits to fully participating in the selected training program and to attending all scheduled sessions.

The Athlete agrees to adhere to the training schedule and to notify the trainer in advance of any absences.

No make-up sessions will be provided if the Athlete misses a session.

The Academy may make concessions based upon travel or other extraordinary circumstances.

7. Exclusivity:

During the term of this Agreement, the Athlete agrees to exclusively train with the Academy and will not seek or engage in training with other coaches or training programs without prior written consent from the Academy.

The Athlete understands that exclusive training ensures consistency and maximizes the effectiveness of the program.

8. Confidentiality:

The Athlete agrees to keep all training materials, methods, and proprietary information confidential and not to disclose them to any third parties without the Academy's written consent.

9. Media Release:

The Athlete grants the Academy permission to use photographs, videos, and other media of the Athlete for promotional and marketing purposes.

10. Governing Law:

This Agreement shall be governed by and construed in accordance with the laws of the state of Texas.

11. Entire Agreement:

This Agreement constitutes the entire agreement between the parties and supersedes all prior agreements or understandings, whether written or oral.

12. Amendments:

Any amendments to this Agreement must be made in writing and signed by both parties.

13. Cancellation Policy

By signing this agreement, the member acknowledges and agrees that there are no cancellations permitted once the membership term commences. The member commits to paying the full term of the contract, as outlined herein.
